Minimum Requirements to correct your Status:

*Verification of living on Indiana today

*Two notarized Witness Testimonies from two separate people over the lawful age of twenty-one and have known you for a minimum of seven years

*Birth Certificate, with any documents that support Name changes

*Declaration of Naturalization Act of 1779
