Phase 1
Witness Testimony – 2/Two separate people, preferably over the Lawful age of 21, that have known you for a minimum of 7 years. You will include your 2×2 photo with a white background shoulders up, your Name, and Indiana home address at the top. Give each witness one. In the middle section, each witness will include their relationship to you, length of time known, a contact, and sign as they normally would in Blue ink in front of a Notary. The Notary will complete the bottom portion and give back to your witness. Each witness will return their signed and notarized template back to you. You will then present each completed Witness Testimony to your Assembly Recording Secretary for Recording and Publishing and then return them to you.
