• Birth Certificate
• Your Parent’s Names and Matrimony Date and Town, State
• Current/Valid Photo ID/DL with Name/NAME coming in and Indiana Home Address. Note: the US Passport is Not requested as it does Not serve to provide an Indiana address.
• Supporting documentation of all Name changes
• Current utility or rent/mortgage/lease/agreement in the Name/NAME coming in and Current Indiana Home address
• Photo – 2×2, clean bright background, front view of face and no ‘selfie’ style view
• Thumbprint – Right in Red ink
• Birth Certificates for all children and/or grandchildren under the lawful age of 21, time when born, Mom’s Name-County and State, or foreign Country, where born, Dad’s Name-County and State, or foreign, Country where born, and address of Baby and Family for each child

Note: Most choose National, however, You may choose Citizen, but cannot Act as a State Citizen without presenting the 928 Bundle in addition to the documents below, along with Federation and Assembly Training, and some roles may require Status Correction documents review and vetting, depending upon service.

An Assembly recording secretary will prepare:

• 928 Bundle:
  • Deed of ReConveyance/Conveyance
  • Certificate of Assumed Name
  • Act of Expatriation
  • Cancellation of Prior Powers of Attorney-can include Exceptions
  • Mandatory Notice – FSIA Foreign Sovereign Immunities Act
  • Paramount Claim of the Life

• Fee Schedule
• Declaration of Naturalization of 1779, either American born or Immigrant-Naturalized, Green Card, 7-years Undocumented (must present 7 years of living on American Land)
• Common Carry Declaration
• Declaration of Political Status-requires crimped seal
• Letters to US Sec. of St., US Asst. Sec. of Consular Affairs, and Birth State Sec. of St. that goes with the Declaration of Political Status
• Letter – Revocation of Election to Pay Taxes to IRS-DC (not applicable if Federal/State/Municipal EMPLOYEE or Active Duty Military)
• Letter – Revocation of Election to Pay Taxes to IRS-NY (not applicable if Federal/State/Municipal EMPLOYEE or Active Duty Military)
• Military Severance Cover Letter Only – blank DD2168 provided for you to complete from DD214, if applicable (if currently active duty, or in service in some form, this has to wait until ‘Time-Served’)
• Baby Deeds will be prepared as well, for children and/or grandchildren – Birth Certificates presented for all children under the lawful age of 21.
